അദ്ധ്വാനിക്കുന്നവരും ഭാരം വഹിക്കുന്നവരുമായ നിങ്ങൾ എല്ലാവരും എന്റെ അടുക്കൽ വരുവിൻ. ഞാൻ നിങ്ങളെ ആശ്വ സിപ്പിക്കാം” എന്ന് അരുളിച്ചെയ്തുകൊണ്ട് ദുഃഖിതരെ തന്റെ പക്കലേക്കു സ്നേഹപൂർവ്വം സ്വീകരിച്ച കാരുണ്യവാനായ കർത്താവേ, രോഗാതുരനും, മരണാസന്നനുമായിരിക്കുന്ന ………എന്ന സഹോദരന്റെ ( സഹോദരിയുടെ ) മേൽ കൃപയാ യിരിക്കണമേ. ജീവിത കാലത്ത് അങ്ങയുടെ നാമത്തിൽ ചെയ്തിട്ടുള്ള എല്ലാ സത്പ്രവൃത്തികളും പുണ്യപ്രവൃത്തികളും പരിഗണിച്ച് മരണനേരത്ത് ഇയാളെ ആശ്വസിപ്പിക്കുകയും അനുഗ്രഹിക്കുകയും ചെയ്യണമേ. അങ്ങയോടുള്ള സ്നേഹത്തെ പ്രതി ഇയാൾ അനുഭവിച്ചിട്ടുള്ള ക്ലേശങ്ങളെയും ബുദ്ധിമുട്ടുകളെയും ഓർത്ത് ഈ സഹോദരന്റെ/സഹോദരിയുടെ മേൽ കരുണയായിരിക്കണമേ. അങ്ങയുടെ രക്ഷാകരമായ പീഡാനുഭവത്തെയും കുരിശുമരണത്തെയും പ്രതി അറിഞ്ഞും അറിയാതെയും ഈ സഹോദരന്റെ (സഹോദരിയുടെ) ജീവിതത്തിൽ വന്നുപോയ പാപങ്ങളെല്ലാം പൊറുക്കുകയും കടങ്ങളെല്ലാം മോചിപ്പിക്കുകയും ചെയ്യണമേ. കർത്താവേ, അങ്ങയുടെ തിരുസഭയുടെ അംഗമായ ഈ സഹോദരനെ(സഹോദരിയെ) അങ്ങയുടെ തിരുരക്തത്താൽ കഴുകി വിശുദ്ധീകരിക്കുകയും അങ്ങയുടെ കുരിശിന്റെ ശക്തിയാൽ എല്ലാവിധ തിന്മകളുടേയും ശക്തികളിൽ നിന്നും സംരക്ഷിക്കുകയും ചെയ്യണമേ. അങ്ങുന്നു തിരു മനസ്സാകുന്നുവെങ്കിൽ ഈ സഹോദരനു ( സഹോദരിക്ക് ) നല്ലൊരു മരണം പ്രദാനം ചെയ്യുകയും അതുവഴി സ്വർഗ്ഗരാ ജ്യത്തിൽ അങ്ങയുടെ തിരുമുഖം ദർശിക്കുവാനും സ്വർഗ്ഗത്തിലെ സകല മാലാഖമാരോടും വിശുദ്ധരോടും ചേർന്ന് അങ്ങയെ നിരന്തരം പാടി സ്തുതിക്കുവാനും അനുഗ്രഹിക്കണമേ. ജീവന്റെയും മരണത്തിന്റെയും നാഥനും നിത്യം പിതാവും പുത്രനും പരിശുദ്ധാത്മാവുമായ സർവ്വേശ്വരാ ആമ്മേൻ.

Come to me, all you who labor and are heavy laden. O Merciful Lord, who lovingly received the sorrowful to Himself by saying, “I will give you hope”, have mercy on the sick and dying brother (sister) of……… Please console and bless him at the time of death considering all the good deeds and pious deeds done in your name during his life. Have mercy on this brother/sister for the sufferings and hardships he/she has gone through for love for you. Please forgive all the sins that have come into this brother’s (sister’s) life knowingly or unknowingly and release all the debts due to your saving suffering and death on the cross. O Lord, wash and sanctify this brother (sister) who is a member of your Holy Church with your holy blood and protect him from all evil forces by the power of your cross. If it pleases you, this brother (sister) Grant me a good death so that I may see Your face in the kingdom of heaven and sing praises to You constantly with all the angels and saints in heaven.
